Reassessment Forms

APPLICATION FOR CHANGED ASSESSMENT  (BOE 305-AH) - This form is used to formally request an assessment change and is to be submitted to the Clerk of the Board of Supervisors, Assessment Appeals Board. 

APPLICATION FOR TAX SEGREGATION (SEPARATE VALUATIONS) - use to request the allocation of split values to facilitate payment of taxes

REQUEST FOR REAPPRAISAL - When the taxpayer believes he/she is being over assessed this form may be submitted to the Assessor's Office so that the current assessed values may be reviewed.

PRELIMINARY CHANGE OF OWNERSHIP REPORT (BOE 502-A) - This document must be completed and filed whenever there is a change of ownership. It's usually filled out in escrow and accompanies the deed when recorded.

CHANGE OF OWNERSHIP STATEMENT (BOE 502-AH) 
CHANGE OF OWNERSHIP STATEMENT - RESIDENT OWNED MOBILE HOME PARK - use this form to report a change in ownership of a mobile home in a resident owned park.
CHANGE OF OWNERSHIP STATEMENT - NOTICE OF DEATH OF REAL PROPERTY OWNER  (BOE 502-D) - File this as a Change of Ownership Report when the assessee has died.  
PROPERTY DAMAGED BY MISFORTUNE OR CALAMITY - Allows adjustments to assessed value for properties damaged through no fault of the owner.

LESSORS' EXEMPTION CLAIM (BOE 263) - To be filed by lessor. Property used for free public libraries and free museums and for property used exclusively for public schools community colleges, state colleges, state universities, University of CA,  churches and nonprofit colleges. The benefit of the exemption must go to the exempt organization in the form of a rent reduction or direct refund, unless otherwise stated in the lease. 
QUALIFIED LESSORS' EXEMPTION (BOE 263-A) - To be filed by lessor. Claim for exemptions claimed on property used for free public libraries and free museums and for property used exclusively for public schools, community colleges, state colleges, state universities, University of California, and  nonprofit colleges. For property which is being purchased with an agreement granting the lessee the option to acquire the leased property for one dollar or other nominal amount at the end of the lease.  
LESSEES' EXEMPTION CLAIM (BOE 263-B) -  Property used exclusively for public schools, community colleges, state colleges, state universities, or University of California.
COLLEGE EXEMPTION (BOE 264-AH) - Claim for exemption from property taxes 
CEMETERY EXEMPTION (BOE 265) - Claim for exemption from property taxes   
CEMETERY EXEMPTION BOE (265-NT)- Change in eligibility or termination notice 
CLAIM FOR HOMEOWNERS' PROPERTY TAX EXEMPTION (BOE 266) - Exempts $7,000 in assessed value (a $70 per year reduction in taxes) from the principle residence of an eligible homeowner. 
RECLAMO PARA LA EXENCIÓN DE IMPUESTOS PREDIALES DE LOS PROPIETARIOS DE CASAS (BOE 266-S) - Se excluye $7,000 del valor asesado (una reducción de $70 cada año) de la residencia principal para los que están elegibles. 
HOMEOWNERS' EXEMPTION TERMINATION NOTICE - use this form to notify the Assessor's Office when you have moved from a property and therefore no longer qualify for the Homeowner's Exemption on that property.

Miscellaneous Forms
ADDRESS CHANGE AUTHORIZATION - Use this form to change the mailing address on your tax bill. It is not used for changing the owner's name.   
AFFIDAVIT OF OWNERSHIP TRANSFER (LEGAL ENTITY AFFIDAVIT) 
AUTHORIZATION TO ACCESS CONFIDENTIAL FILES - This completed form allows the designated person to view confidential property files in the Assessor's Office. 
COMBINATION/SPLIT REQUEST FORM - This is an owner-initiated request to combine more than one parcel into a single parcel. (See our brochure entitled Split/Combination/LBA: Where Boundaries Change under Tax Cycle Information.)  
PROPERTY USE REPORT - This is not a claim form - to be filed along with any other form(s) necessary to make eligible for exemption 
SWORN STATEMENT TO ASSESSOR (DECLARATION REGARDING TRANSFER OF PROPERTY) - This affidavit is used to explain the exact circumstances of a transaction so that the Assessor may properly determine whether a particular event is re-appraisable or not.
